Please note that the general feedback form does not replace any formal grievance, appeal, discrimination complaint, or other procedure that may apply to a particular concern.

Filing a Civil Rights Complaint with the U.S. Department of Health and Human Services

Depending on the nature of your concern and the program involved, you may also have the right to file a civil rights complaint with the U.S. Department of Health and Human Services (HHS), Office for Civil Rights (OCR).

HHS OCR accepts complaints alleging discrimination by covered health and social service programs or organizations receiving applicable federal financial assistance.

Civil rights complaints generally should be filed within 180 days of when you knew that the alleged discrimination occurred, although HHS OCR may extend the filing period when good cause is shown.

No Retaliation

Opportunities, Inc. prohibits retaliation against a person for raising a good-faith concern about discrimination, requesting an accommodation or assistance, providing feedback, or participating in an applicable complaint or review process.
